Abstrakty

Artykuł zawiera omówienie cech samorządowego modelu ochrony zdrowia oraz wnioski z obserwacji jego funkcjonowania w krajach skandynawskich i w Hiszpanii. Model samorządowy charakteryzuje: dominacja publicznych źródeł finansowania, w znacznej części pochodzących z budżetów samorządów lokalnych, dysponowanie środkami finansowymi na ochronę zdrowia przez władze i administracje lokalne, samorządowa własność większości zakładów opieki zdrowotnej oraz powszechny dostęp obywateli do usług zdrowotnych. Na tle charakterystyki wydatków na ochronę zdrowia i zasobów autorka przedstawia systemy zarządzania ochroną zdrowia w poszczególnych państwach, z zaznaczeniem podobieństw i różnic. Szczególną uwagę zwraca na regulacyjne funkcje państwa oraz problematykę integrowania opieki zdrowotnej w sytuacji decentralizacji uprawnień wielu ośrodków decyzyjnych.

The article offers a description of characteristics of a local governmental model of health care. According to the author, these characteristics are: the domination of public sources of financing (especially local governments' budgets), the distribution of money on health care by the local government and administration, local government's ownership of most health-care units, availability of health-care services to every citizen. The article describes local governmental health care in the Scandinavian countries and Spain, presenting similarities and differences between particular systems and the reforms they have gone through. Special note was taken of the regulative function of the state and the problems of integrating health care in the situation of multiple and decentralised decision-making institutions. As important factors of the proper functioning of a local governmental health care the author listed: a high professional and ethical level of local government and administration officers, culture of negotiating, evolutionary way of changing the system, emphasis on patients' rights, dealing with health problems concerning the local community, application of new instruments of financing, control, and organisation.
