The Future of Disability in Poland

In this paper we use demographic models to project future needs of long-term care in the Poland, based on estimates of the prevalence of disability in the SHARE database, and combine these with the EUROPOP2008 mortality forecast. Long-term care need is operationalised as at least one limitation in activities of daily living (ADL-disability), based on the Katz ADL disability scale (Katz et al. 1963). ADL-disability is defined as self-reported difficulty with any of the following items: (a) bathing, (b) dressing, (c) eating, (d) indoor transferring and (e) toileting and continence. As generally the ADL items are rather hierarchical, a mean score on these items is a good indication of severity. We also examined the evolution of mortality in Poland and compared these with neighboring Germany, estimated the prevalence of disability in SHARE, made disability projections for Poland and compared these to Germany. Artykuł poświęcony jest oszacowaniu potrzeb związanych z opieką długookresową na podstawie prognozy liczby osób starszych ze znacznym upośledzeniem sprawności do 2040 r. w Polsce. Szacunki te zostały porównane z sytuacją w Niemczech. Dostępne dane statystyczne sugerują wysoki poziom niepełnosprawności w Polsce. Scenariusz prognoz przewiduje starzenie się populacji powojennego baby boomu i zakłada wzrost przeciętnego trwania życia wraz z odwlekaniem w czasie okresu spędzonego w niepełnosprawności. Przy takich założeniach liczba osób starszych dotkniętych niepełnosprawnością wzrasta o 92%. Jeśli poziom niepełnosprawności w Polsce byłby zbliżony do obserwowanego w krajach Europy Zachodniej, wówczas wzrost ten byłby mniejszy (62%), co pokazuje znaczny potencjał związany z prewencją niepełnosprawności, leczeniem i rehabilitacją niepełnosprawnych osób starszych.
