Abstrakty

Niniejszy artykuł stanowi przegląd literatury i na tej podstawie próbę określenia stopnia konwergencji wydatków na opiekę zdrowotną w państwach Unii Europejskiej. W przypadku systemów opieki zdrowotnej, których kształtowanie w poszczególnych państwach Unii Europejskiej zdeterminowane jest zróżnicowanymi uwarunkowaniami demograficznymi, historycznymi, politycznymi i gospodarczymi, proces konwergencji wydaje się być mocno skomplikowany. Niemniej, na przestrzeni czasowej lat 1970-2014, zidentyfikowano 10 publikacji odpowiadających określonemu zakresowi prowadzonych analiz. Dokonana analiza pozwoliła na wciągnięcie wniosków, co do dalszych tendencji rozwojowych w kształtowaniu się zarówno publicznych, jak i prywatnych wydatków na opiekę zdrowotną w państwach Unii Europejskiej. Przedstawiony przegląd literatury dotyczącej konwergencji wydatków na opiekę zdrowotną wydaje się sugerować, iż w państwach Unii Europejskiej następuje proces zbieżności w systemie finansowania opieki zdrowotnej. Faktem jest, iż dynamika i siła konwergencji zależy od przyjętego do badania zakresu czasowego i przestrzennego, przyjętych definicji konwergencji oraz zastosowanej metodologii ekonometrycznej. Badania potwierdzają jednak, że poziomy wydatków na opiekę zdrowotną wykazują tendencję do spadku stopnia zróżnicowania (szczególnie w państwach o wyższym poziomie dochodu). Zbieżność wartości wydatków może być przy tym wynikiem konwergencji systemów ochrony zdrowia na podłożu otwartej koordynacji. Możliwość zastosowania przez szczebel ponadnarodowy innych instrumentów wspierających dynamikę ujednolicania systemów opieki zdrowotnej jest mocno ograniczony - ze względu na poszanowanie suwerenności poszczególnych państw i obowiązującą zasadę subsydiarności. This article is a review of the literature and an attempt to determine the degree of convergence of health care expenditure in the European Union. In the case of different European health care systems convergence process seems to be very complicated. The case is that the national health care systems are determined by diverse demographic, historical, political and economic circumstances. However, over the years 1970-2014, 10 publications corresponding to a particular range of analyzes are identified. The analysis allowed to make conclusions about further developments in the formation of both public and private spending on health care in the European Union. An overview of the literature on the convergence of health care expenditure seems to suggest that in the EU convergence process takes place in the system of health care financing. The fact is that the dynamics and strength of convergence depend on the time scope and spatial range adopted to the study as well as accepted definitions of convergence and the econometric methodology used. Research confirms, however, that the levels of spending on health care tend to decrease the degree of differentiation (especially in countries with a higher level of income). The convergence of expenditure may be the result of the convergence of health systems on the ground of open coordination method. The applicability of the supranational level of other instruments supporting dynamic unification of health systems is very limited - because of the respect for national sovereignty and the principle of subsidiarity.
