The purpose of the article is to present the FOCUS project (Frailty Management Optimization through EIP AHA Commitments and the Utilization of Stakeholders Input) funded under the 3rd Health Program of the European Commission. The article focuses on the impact of the cooperation among consortium partners and the aspects of the research results' dissemination. FOCUS is implemented in the international consortium that brings together an interdisciplinary team of specialists from 10 organizations in 6 countries: Uni- versitat de Valencia (Spain, project coordinator), Roessingh Research and Development BV (Netherlands), Uniwersytet Medyczny im. Piastów Śląskich we Wrocławiu (Poland), Aston University (United Kingdom), Fondazione Irccs Ca' Granda - Ospedale Maggiore Policlinico (Italy), Istituto di Ricerche Farmacologiche Mario Negri (Italy), Escola Superior de Enferma- gem de Coimbra (Portugal), Universidade de Aveiro (Portugal), Everis Spain SL (Spain), Estu- dios de Software Avanzado y Mantenimiento de Tecnologia Sociedad Limitada (Spain) that work together on the particular scientific research problem. Using the case study method, the authors show that the assimilation of the diversity of the research institutions in the consortium allows scientific and economic challenges to be overcome and facilitates the effective transfer of knowledge from the world of science to the medical practice.
