Outpatients clinics and hospitals are aggressively redesigning their organisations through the strategy of systems redesign. System redesign reąuires the organisation to rethink and redesign traditional structure, work roles, critical Systems, and related processes used to produce, deliver, or support patient care (Moss et. al., 1994). Prior to the initiation of a formalised programme of systems redesign, many organisations have done cost reductions, utilizing such strategies as hiring freezes, position eliminations, and reductions in budgets. Many such strategies are carried out without measuring the impact on productivity, quality, and efficiency of the deliyery of care. Effectiveness raises issues of customer expectations and while most people want to achieve their expectations at the lowest possible cost, they usually are not willing to severely affect outcomes solely to achieve reduced costs. Effec- tiveness is also an important concept that must be included in any conversation about outsourcing, privatising, downsizing, or reducing levels of service. Com- petitive bidding processes must assure a level playing field regarding the expec- tations of customers. The performance measures used to evaluate the effective- ness of the programme in meeting its strategie organisational objectives must be elear. Sometimes it may make morę sense to eliminate a programme altogether rather than to out the costs (by reducing the inputs) below the level that provides for a minimum level of effective service delivery.
