One of the important determinants of population health is an adequate access to health care services. Information about health care which was seen by people as needed, but not appropriately received may be provided by indicators of unmet health care needs (UHCN). In many cases the access to health care services in rural areas is lower than in urban areas. The aim of this study is to analyse UHCN in the European Union countries in the context of a degree of urbanisation, with the particular reference to Poland. The study was conducted for 26 countries and was based on self-reported information obtained from the European Health Interview Survey. The analysis showed that looking at UHCN generally, without specifying a single reason, a share of people reporting UHCN is higher in cities than in rural areas in the vast majority of countries. The same relations may be observed for the UHCN analysed separately by reasons and caused by financial issues and by waiting lists. Only the UHCN due to distance/transportation problems are reported more frequently in rural areas. A deeper understanding of the mechanisms of UHCN differences and the factors affecting them would require a more detailed analysis, based on a more extensive database.(original abstract)

Jednym z ważnych wyznaczników czynników wpływających na zdrowie populacji jest odpowiedni dostęp do opieki medycznej. Informacje na temat opieki zdrowotnej, która była postrzegana przez ludzi jako potrzebna, ale nie została odpowiednio zaspokojona, mogą być dostarczone przez wskaźniki niezaspokojonych potrzeb zdrowotnych (unmet health care needs - UHCN). W wielu przypadkach dostęp do świadczeń opieki medycznej na obszarach wiejskich jest niższy niż na obszarach miejskich. Celem niniejszego opracowania jest analiza UHCN w krajach Unii Europejskiej w kontekście stopnia urbanizacji, ze szczególnym uwzględnieniem Polski. Badanie zostało przeprowadzone w 26 krajach i opierało się na informacjach pochodzących z Europejskiego Ankietowego Badania Zdrowia (EHIS). Analiza wykazała, że patrząc na UHCN ogólnie, bez określania pojedynczej przyczyny, odsetek osób zgłaszających UHCN jest wyższy w miastach niż na obszarach wiejskich w zdecydowanej większości krajów. Te same zależności można zaobserwować w przypadku UHCN analizowanych oddzielnie według przyczyn i wynikających z kwestii finansowych oraz list oczekujących. Jedynie UHCN spowodowane odległością/problemami transportowymi są częściej zgłaszane na obszarach wiejskich. Głębsze zrozumienie mechanizmów różnic UHCN i czynników na nie wpływających wymagałoby bardziej szczegółowej analizy, opartej na obszerniejszej bazie danych.(abstrakt oryginalny)
