Abstrakty

Za cel artykułu przyjęto przeanalizowanie zdrowotnych wydatków publicznych, gdyż często punktem wyjścia dla podejmowania decyzji dotyczących zarządzania w polskim systemie zdrowotnym jest określenie tendencji tych wydatków. Ten artykuł skupia się na analizie przestrzenno-czasowej wspomnianych wydatków dla wybranych krajów UE, jako regionów z których doświadczeń korzysta Polska prowadząc politykę zdrowotną. Ponadto dokonano analizy: udziału wydatków publicznych w PKB oraz zdrowotnych wydatków publicznych i dobrowolnych prywatnych w całkowitych. Tutaj przeanalizowano kształtowanie się wydatków zdrowotnych według Systemu Rachunków Zdrowia. Zagadnienie rozpatrywano od strony ekonomicznej dlatego wydatki zdrowotne uzależniono tylko od głównego miernika wzrostu gospodarczego - PKB. Badania przeprowadzono na podstawie danych rocznych za lata 2013-2020 w oparciu o bazę danych Eurostat-u, a do ukazania tendencji wykorzystano dane przekrojowo-czasowe. We wszystkich krajach PKB to istotna determinanta wydatków zdrowotnych, a opieka zdrowotna jest postrzegana jako dobro luksusowe w długim okresie, zaś w krótkim jako dobro pierwszej potrzeby. Najwięcej na zdrowie, ze źródeł publicznych, przeznaczają: Malta, Słowenia i Czechy, a wśród krajów CEE w latach 2013-2019 Polska ma większe wydatki tylko od Łotwy, natomiast w 2020r. zajmujemy ostatnią pozycję (4,69%PKB). W latach 2013-2020 w prawie wszystkich badanych krajach bardzo mało wzrósł udział wydatków publicznych w całkowitych - najmniej w Rumunii, Estonii, Polsce i Słowenii. To świadczy o braku zaspokojenia potrzeb pacjentów przez publiczną służbę zdrowia, co wyraża się wzrostem zainteresowania dobrowolnymi ubezpieczeniami zdrowotnymi. Z tego względu uchwycenie zmian opisywanych wydatków ma niebagatelne znaczenie, a ważność analizy wydatków zdrowotnych jest związana ze stanowieniem przez nie coraz większej części PKB. Nadto, międzynarodowe porównania pozwalają na efektywniejsze reformowanie ochrony zdrowia i są podstawą dla decydentów przy zarządzaniu tą sferą. (abstrakt oryginalny)

The purpose of this article was to analyse the public health expenses as the specification of these expenses tendency is often a beginning point to make decisions referring to the management in the Polish healthcare system. This paper focuses on the space and time analysis of the mentioned expenses for the selected EU states being the regions the experience of which Poland uses leading its healthcare policy. Moreover, the participation of public expenses in GDP and public and voluntary of private healthcare expenses in total expenses was analysed. Shaping the healthcare expenditure was analysed according to the Health Accounting System. The issue was considered from the economic point of view therefore, the health expenses depended only on one economic growth index - GDP. The research was conducted on the basis of the annual data for years 2013 - 2020 on the basis of the Eurostat database and the cross-sectional data were used to show the tendencies. In all countries GDP is a significant determinant of health expenses and the healthcare is perceived as a luxury goods in the long term and as a necessity in the short term. The states that incur the highest health expenses, from the public funds, are: Malta, Slovenia and the Czech Republic and among the CEE states in years 2013-2019 Poland had higher expenses only than Latvia and in 2020 we took the last position (4.69% of GDP). In the years 2013-2020, the share of public expenditure in total expenditure increased very little in almost all surveyed countries - the least in Romania, Estonia, Poland and Slovenia. This trend shows that the public healthcare does not satisfy the patients' needs which is visible by the increase in the interest in voluntary medical insurance. Due to this fact, it is extremely important to notice the changes of the described expenses and the significance of the health expense analysis is connected with the fact that they make bigger and bigger part of the GDP. Moreover, the international comparisons enable to reform healthcare more efficiently and are the basis for deciding parties to manage this area. (original abstract)
