The role of knowledge management and telehealth in patient self-management of chronic disorders: an ontological approach

Recently, many self-management systems are implemented using computerized information systems and various telehealth approaches such as phone calls, teleconferencing, e-mails, chats, and Web-based educational modules. The patient self-management process is often divided into three groups of tasks: medical management, life-role management, and emotional management. This paper takes an integrative approach to these tasks and concentrates on patients' mental health as essential factor in the success of chronic disorders self-management. It proposes an ontological framework for knowledge management and for the communication process in patient-focused care. The ontological model addresses the need for data, information, and knowledge interoperability in patient self-led care. This model, based on the World Wide Web Consortium's standard Web Ontology Language, OWL, identifies the necessary concepts required to coordinate and facilitate mental care and emotional support.
