Abstrakty

W świetle toczących się dyskusji na temat przyszłości polskiego systemu ochrony zdrowia warto na podstawie badań literaturowych podjąć próbę znalezienia rozwiązania głównego dylematu, który brzmi - ile w finansowaniu ochrony zdrowia powinno być udziału państwa, a ile rynku? Po przeanalizowaniu różnych koncepcji wydaje się, że optymalnym rozwiązaniem jest zastosowanie tzw. rynku wewnętrznego. Dokonują oceny tej koncepcji, należy uwzględnić takie zjawiska, jak: selekcja negatywna, hazard moralny i koszty transakcyjne oraz wskazać sposoby rozwiązywania tych problemów. (fragment tekstu)

The article presents some arguments for and against an active role of the country in financing health protection. As an alternative the internal trade has been suggested. In this case, the role of the country as a payer is taken over by insurance companies. The paper contains both the evaluation of the conception and the presentation of the problems connected with financing health care services by those companies. The article includes some phenomena such as a negative selection, a moral hazard and the costs of the transactions. Moreover, social insurances have been described as a way of reducing a negative selection. In addition, the article presents the examples of the hindrances concerning both supply and demand which help to solve two other problems. The experiences of the United States of America in solving problems of financing medical services have been discussed as well. (original abstract)
