Abstrakty

Artykuł przedstawia podstawowe dylematy 'odwiecznej' debaty o roli rynku i państwa w ochronie zdrowia w nawiązaniu do pakietu ustaw zdrowotnych przyjętych w Polsce w latach 2010-2011, zwiększających udział prywatnej własności i komercyjnych zasad w funkcjonowaniu placówek zdrowotnych. Przypomina teoretyczne tezy z jednej strony o zawodności rynku, a z drugiej o zawodności państwa w dostarczaniu usług zdrowotnych. Wskazuje kierunki reform sektora zdrowotnego, niwelujących zawodności obu skrajnych systemów i wdrażających rozwiązania łączące zasady solidaryzmu i swobody wyboru oraz indywidualnej odpowiedzialności. Zwraca uwagę na konieczność spełnienia określonych warunków, aby wprowadzane zmiany mogły być efektywne zarówno ze względu na kryteria zdrowotne i społeczne jak i ekonomiczne. (abstrakt oryginalny)

The article discusses the main dilemmas of the debate on the role of market and state in providing health care. The discussion is held in the context of the health reform package introduced in Poland in 2010-2011, which increased the share of private sector and commercial rules in health care institutions. The author recalls theoretical arguments about market and state failures in providing health services. She indicates the desirable directions of reforms in the health sector that would reduce the shortcomings of both extreme systems, introducing the arrangements that would combine the principles of solidarity, free choice, and individual responsibility. The author specifies the conditions, which have to be fulfilled in order to assure that changes introduced by reforms turn to be effective both in terms of social and economic criteria.
